Hemodynamic studies in a case of carcinoid cardiovascular syndrome.

چکیده
NUMEROUS reports of cases of malignant carcinoid associated with acquired cardiovascular abnormalities have appeared in the medical literature since this syndrome was first recognized as a clinical entity. Despite the fact that the intracardiac abnormalities characteristic of this interesting disease are almost exclusively confined to the right side of the heart, catheterization studies have only infrequently been mentioned and no truly complete hemodynamic data have been included in the case reports published to date. Of the valvular lesions encountered in the carcinoid cardiovascular syndrome, tricuspid insufficiency is the most frequently seen; right atrial tracings exemplifying this condition appear in the communications of Bjorck, Axen, and Thorson' and Sjoerdsma, Weissbach, and Udenfriend.2 However, cardiac catheterization data supporting the diagnoses of tricuspid stenosis, pulmonary stenosis, and pulmonary insufficiency, which may also occur in association with functioning carcinoid, have not come to our attention. Serotonin or 5 hydroxytryptamine (5-HT), the elaboration of which by the argentaffine cells has been generally accepted as the pathogenic agent responsible for the cardiovascular manifestations of malignant carcinoid, has been shown to produce pulmonary hypertension in animals.3 The occurrence of pulmonary hypertension in the reported cases of carcinoid cardiovascular disease, however, has not been frequent. Sparks and Towbridge4 reported that cardiac catheterization established the diagnosis of pulmonary hypertension in their case but the

Cushing’s syndrome associated with typical, peripheral pulmonary carcinoid tumor and N2 lymph node metastasis: case report
Background: The bronchopulmonary carcinoid tumor accounts for 1-2% of all adult malignancies of the lung and 20-30% of all carcinoid tumors. Cushing’s syndrome is the result of chronic exposure to increased concentration of exogenous or endo-genus cortisol hormone, and it is generally associated with central obesity, metabolic syndrome, and hypertension. Treatment is based on decreasing cortiso...
متن کاملCushing syndrome secondary to thymic carcinoid synchronous with tuberculous lymphadenitis and pulmonary tuberculosis: a case
Abstract Thymic carcinoid is an uncommon neoplasm that can present with Cushing syndrome. We report a 39-year old woman with symptoms of Cushing syndrome secondary to thymic carcinoid and synchronous with tuberculous lymphadenitis and pulmonary tuberculosis.
متن کاملAnesthetic considerations and management of a patient with unsuspected carcinoid crisis during hepatic tumor resection.
Anesthetic management for massive blood loss in liver surgery concomitant with hemodynamic instability secondary to carcinoid crisis can be challenging in the perioperative setting. Hypotension, diarrhea, facial flushing, bronchospasm, and tricuspid and pulmonic valvular diseases are the common manifestations of carcinoid syndrome. This report illustrates the importance of early recognition and...
متن کاملHeart Carcinoid Disease with Patent Foramen Ovale Treated by Mini Sternotomy
This case report was an attempt to present the importance of diagnostic echocardiography and closure of a patent foramen ovale in carcinoid heart disease. Herein, we present a case of a 48-year-old woman with a carcinoid metastasis presenting with tricuspid regurgitation, pulmonic stenosis, patent foramen ovale, and borderline left ventricular ejection fraction on an echocardiogram. Surgical in...

Carcinoid tumors are slow growing neoplasms of neuro-endocrine tissues arising from enterochromaffin cells and although rare can create serious problems during the surgery due to myriad clinical manifestations and perioperative complications. A slow growing tumor, it remains asymptomatic and is mostly found coincidentally intraoperative. Tumor secretes mediators which, if, bypass the hepatic me...
